Payment is accepted through cash, credit card via **PayPal, insurance and EAP. **If you wish to pay for services with credit card, please pay the day of your session by clicking on the Pay Now button below which is through Paypal.
Cancellations: If not given within 24 hours there is a $50 fee.
Insurance: Please contact your insurance company before calling to find out if I am “In Network” and what your co-pay will be. If I am not “In Network” as a provider ask your insurance company if you have “Out of Network” benefits and if so what your cost will be to me. I am In Network with: Anthem, Aetna, Prime Care (OSU), Medical Mutual, Tricare and United Health Care (Optum).
*It is the client’s responsibility to verify insurance coverage and the client is ultimately responsible for payment.
I am NO longer taking any Medicaid clients. This includes Molina and Caresource. For Medicaid, Molina and Caresource I would refer you to Access Ohio.
EAP: Contact your Human Resources department to see if your company or agency has an Employee Assistance Program. They will give you a 1-800 number to call for an intake. The EAP will then give you choices of providers where you can go in your area for psychotherapy. They will also give you an Authorization Number (and tell you how many sessions you can have) which you need to give me when you call.
DOCUMENTATION FEE: $25 for disability paperwork or paperwork needed for attorneys, doctors, etc… provided that I have a release of information signed and on file ahead of time.
Other Fees: If I am needed for other services such as making an appearance in court, the charge is my full on-going fee per hour . My full fee (not insurance rates) is noted in your Informed Consent that is signed at the time we begin service.
